The UCLA Celiac Disease Center, with the endorsement of the Celiac Disease Foundation (CDF), has developed an innovative online registry. This will allow them to better understand the natural history of gluten-associated disorders, specifically celiac disease, and identify barriers for being compliant with a gluten-free diet.
By completing a confidential online survey you can help them advance the research in the field of gluten-associated disorders and see how these disorders affect our community as a whole in terms of health care and quality of life. The data collected as part of the UCLA CELIAC COLLECTIVE will provide useful information and help the UCLA Celiac Disease Center to better understand gluten-associated disorders related behaviors and health.

this is a great survey, seems like they really understand the emotional nuances around receiving a diagnosis and the difficulty (at times) of following a strict GF diet.
